Most concrete failures are base failures. The pour gets blamed because the pour is what you can see.
We strip organics, bring in the right base material rather than whatever is closest, and compact in lifts instead of one deep pass that only firms up the top few inches. Where a test is warranted we will say so.
Grading takes water away from the building. Getting that right at the start is cheaper than every other way of dealing with water later.
